An assessment is carried out prior to admission when suitability of the service can be gauged. This establishes required needs, wishes and aspirations of each individual, facilitating efficient planning and organisation of the optimum care package.

 

Risk Assessments and Care Plans can be formulated at this stage which will allow for structure and routines which will enable the service user to quickly feel settled.

 

INTRODUCTORY VISITS

An introductory visit will be arranged for the individual and their family or representative. During the visit discussions will take place regarding the service users care and a tour around the home will be offered. Following the visit, it may be advantageous to have a second visit or even an overnight stay. This gives the opportunity to meet other service users and staff, discuss areas of interest or need and to get a general feel of the home. The length of this transition period is based on the needs of the individual and is therefore arranged on an individual basis.

 

TRIAL PERIODS

Admissions will generally take place on a three-month trial period, during which an evaluation of the individuals care needs will be carried out. Each service user is encouraged to take part in this process so that an effective Person Centred Plan is arranged, incorporating required needs, likes and dislikes.

 

The trial period will allow for an assessment of the suitability of the service for the individual and also accommodates for any "change of mind". At the end of the trial period a Care Review will be arranged where it will be decided if the service is appropriate by all concerned. After the initial assessment regular evaluations will be made independently on the care needs of the individual. These will involve the service user and any professionals as appropriate.
