Supported Living

Potens operates five Supported Living projects on the Wirral Peninsula with group houses, shared and single flats, enabling adults with learning disabilities and mental health issues to maintain their independence with the appropriate individual level of support and care to facilitate inclusion in the local community.
The Wirral Peninsula offers easy access to extensive local tourist and historical attractions within the surrounding areas of Liverpool and Chester and indeed has much to offer by way of beaches, countryside & local amenities, with an excellent public transport network.
Tenancies are offered on an assured basis which affords the tenants increased rights and security regarding their accommodation.
Tenants of both sexes are welcome and a wide range of needs, including complex needs and challenging behaviours, can be met. Support and care are provided at an individual level with the personal plan agreed following thorough assessment with the involvement of the service user, professionals involved in their care and - with the service users agreement - family, friends and others involved in their life. We continue to work closely with service users, multi-disciplinary teams (psychiatrists, social workers, community nurses etc.) throughout their time with us, including regular reviews.
The skilled staff teams promote independence and community integration through team, individual and keyworking systems and utilise their wide range of knowledge and experience in enabling individuals to achieve their personal goals. College courses, employment and social activity are promoted and supported by the staff teams who focus on the development of an individuals abilities and strengths.
The care provided within the services is audited by CSCI in line with Domiciliary Care National Minimum Standards and the management team of the Domiciliary Care Manager and four Service Co-ordinators, ensure that the quality of service delivery is maintained and that an out of hours ‘on call' facility is available to offer staff at all levels additional support where this is required.
