Philosophy of our Person Centred Care
Potens has an effective "Life Plan" tool that ensures service users are fully included in all of the decisions made in their lives. Our Person Centred Plans are compiled to cater for the intellectual, emotional, spiritual and social provision that each individual requires.
Staff are aware of the importance of attending to these needs in a way that maintains dignity, respect, freedom from boredom and which prevents insecurity and isolation. We are committed to providing a person centred service in partnership with the individual. The "working partnership" involves four main principles.
TRUST
When an individual is admitted to one of our services this period is recognised as a transition phase in which the service user is adjusting to their new environment. Each person is assigned a Keyworker whose role is to closely work with the individual to ease their anxieties and to learn about their expectations. The Keyworker establishes a professional working relationship with the service user based on honesty and an explicit understanding of how the partnership will function.
POSITIVE REGARD
We respect each and every person and value the right of each individual to make their own decisions. It is with positive regard that we invite the service user to assist in the designing of their own Person Centred Plan. This incorporates a ‘likes and dislikes' list, Daily Activities Programme, Targeted Behaviour Guidelines, and ‘long & short-term goals'. The Person Centred Plan is unique to each individual and is reviewed on a regular basis. Our aim is to make each service user as autonomous as they want to be.
SHARED LEARNING
Working together in partnership means that both staff and service user learn from each other experientially. The knowledge that each acquires effectively strengthens the partnership and ensures that our working practice is regularly monitored, reviewed and evaluated.
EQUALITY
All service users are regarded as individuals who are valued equally.
