Potens services have been developed to maximise an individual’s independence through the referral and assessment process.
Referrals are invited from those wishing to receive services or their representatives. Referrals or enquiries for further information can be made by:

Assessments
The assessment process is individualised with no pre determined length. The information shared during this process will form the basis of the personal support plan and together with introductory visits (including induction periods of up to 12 weeks as appropriate) allow prospective service users to make an informed decision about whether they would like to receive our services.
